Rehabilitation is provided by licensed therapists who are all specially trained to assist each patient in attaining his or her goal. No step toward rehabilitation is taken alone. Through the combined efforts of our therapists, our physicians, our nurses, the patient and his or her family, we develop an individualized therapy program designed to help the patient reach and maintain his full potential. We want your stay in our Transitional Care Unit to be as comfortable and homelike as possible. Our warm, spacious rooms have complimentary in-house televisions with remote control, private telephones, daily local newspaper delivery, private refrigerators and personal laundry service. Discharge Planning begins the day you walk in the door. The Social Services Director works with the patient, family, staff and physician to determine a discharge date. |







The Transitional Care Unit is for patients whose goal is to return home, to the community or to a higher level of functioning. These patients have complex medical conditions that no longer require acute hospitalization, but still need intensive therapy by highly specialized personnel. The clinical therapy services are appropriate for individuals disabled from a variety of conditions including trauma, stroke, chronic pain, amputation, hip and knee replacement, and other disabling diseases. When the patient is admitted to the Transitional Care Unit we immediately assess him or her and begin the rehabilitation process.
